Range

Individual: The individual is the person requiring care or support. An advocate may act on behalf of an individual.

• Explain how the assessment and planning process can be adapted to maximize an individual’s ownership and control of it.

Learning Outcome 2:

• Establish a partnership approach to assessment with the individual
•  Establish how the assessment process should be carried out and who else should be involved, with the individual and others
•  Agree with the individual and others on the intended outcomes of the assessment process and care plan

Range

(2.3) Care plan: A care plan may also be known by other names, such as a support plan, or individual plan of care delivery plan. It is the document where day-to-day requirements and preferences for care and support are detailed.

•  Ensure that assessment takes account of the individual’s strengths and aspirations as well as needs
• Work with the individual and others to identify requirements and preferences to support their well being

• Contribute to an agreement on how component parts of a plan will be delivered and by whom

Learning Outcome 4: 

• Carry out the plan of care or support within the limits of own role
• Lead others to carry out their responsibilities when implementing a plan of care or support
• Adjust the plan in response to changing needs or circumstances

Learning Outcome 5 

• Agree on methods for monitoring the way a care plan is delivered
• Collate and record monitoring information from agreed sources and agree on any changes
• Seek agreement with the individual and others about who should be involved in the review process and the criteria to judge the effectiveness of the care plan
• Seek feedback from the individual and others about whether the objectives of the plan have been met
• Work with the individual and others to agree on any revisions to the plan

• Closing the plan if all objectives have been met
• Reducing the level of support to reflect increased independence
•  Increasing the level of support to address unmet needs
