The fields may include ‘Name of Covered Entity’, ‘State’, ‘Covered Entity Type’, ‘Individuals Affected’, ‘Breach Submission Date’, ‘Type of Breach’, ‘Location of Breached Information’, ‘Business Associate Present’, ‘Web Description’ etc. Data Formatting: For each reported breach, create a separate JSON file.
