These involve professionals from different backgrounds, such as nursing, social workers, occupational therapists, doctors, and healthcare assistants, working together to address the complex needs of person-centred Care.

For instance, this exemplifies how I engage in inter-professional practice within my organization. I am employed by a charitable institution operating under the APS, providing tailored and pragmatic assistance through our educational, housing, justice, and health well-being services. Our team comprises professionals with specialized training and expertise in working with children and young individuals. This multidisciplinary team includes service leads, social services personnel, representatives from local authorities, psychologists, consultant child and adolescent psychiatrists, forensic specialists, specialist nurses, primary mental health workers, child psychotherapists, and counselors.

I provide information, advice, and guidance concerning issues related to criminal records, aiding them in comprehending regulations and asserting their rights during record disclosure. I plan and deliver an adequate support and safety plan to the service users, referring them to specialised agencies as required. I ensure that successful housing benefit claims are submitted. Also monitor the account, record all activities, and take appropriate actions according to the policies and procedures.
